Saint Alphonsus CYO Feedback Form for Parents

 Please help us evaluate your coaches' performance. Complete the evaluation, listing any comments or concerns you may have regarding the coaching staff, the Saint Alphonsus program, or the CYO league. This evaluation allows you the opportunity to share your feelings directly with the athletic board.

The coach treated each player as an individual, remembering the wide range of emotional and physical development for the age group.
The Practices were well organized.
The coach demonstrated knowledge of the rules of the sport and was able to teach these rules to the players.
The coach placed the emotional and physical well-being of the players ahead of any personal desire to win.
The coach demonstrated good sportsmanship at all times toward referees, coaches, and players.
Any disrespect from players toward the facility, coaches, referees, opponents, and teammates was dealt with immediately.
The coach provided equitable playing time for each child. The coach supported a policy whereby high levels of equal participation are emphasized in "Parish" leagues, while allowing more competitive playing time in "Varsity" and "Junior Varsity" leagues.
The coach exhibited a Catholic philosophy of coaching
The coaches/players won graciously and lost with dignity as representatives of Saint Alphonsus Parish.
